If you have booked an Appointment with us, this document gives you some useful information about how to prepare and what to expect.
These are our standard terms and conditions and we will ask you sign a copy of these before a course of treatment can commence.
Treatment of your pet is on a referral basis so this form will need to be completed (and signed by your vet) so that we can share case notes with your vet and report any changes in your animal's condition.
